Receiving Emails from Website Form for new customers a month late!

What can be done to fix this issue??  I have just received two back to back emails this afternoon generated from my website customer form.  They are both back from June!! And the date they needed their service has already passed and I'm sure they thought I had ignored them.  that is not my practice and I definitely do not want any customer to think that of me or my business!  

I have also recently received one as usual with no problem.  So why am I getting brand new emails today with form submissions requesting info from back in June??  Please help and I'd like to know what the glitch is or was that is causing this and if it is fixed.
